Life and Disability Benefits

Basic Life/Accidental Death and Dismemberment (AD&D) benefits are provided at no cost to you. You also have the option to purchase additional supplemental coverage. 

The Hartford administers our Life and AD&D benefits. 

Basic Life and AD&D

  • Basic Life:  2x annual base salary

  • Basic AD&D:  2x annual base salary

  • Plan Maximum: $425,000

  • 100% paid by Enjoy

Voluntary Dependent Life/AD&D

  • Spouse Benefit  : Increments of $5,000 up to $300,000

  • Spouse Guaranteed Issue: $50,000

  • Child Benefit  : $5,000 or $10,000 (birth to 14 days - $0; 14 days to 6 months - $500)

  • 100% paid by you through payroll deductions

Employee Voluntary Life/AD&D

  • Employee Benefit: Increments of $10,000 up to lesser of 5x annual base earnings or $600,000

  • Guaranteed Issue: $300,000

  • 100% paid by you through payroll deductions

1. Employee must elect employee voluntary benefits to elect voluntary benefits for spouse and child(ren). In addition, spouse voluntary life amount cannot exceed 100% of the employee's voluntary life amount.

Evidence of Insurability Process for

Voluntary Life Benefits with The Hartford

If an employee or spouse/domestic partner needs to complete Evidence of Insurability (EOI) an email will be sent to your @enjoy.com email address with instructions on how to go online to complete the process. The email from @thehartford will come to you whether the EOI is for you or your spouse/domestic partner with instructions on how to securely log into the system and provide responses to the medical questions.

 

Upon submission, you will know immediately if you are Approved, Declined or Pended for additional information. If additional information is requested, you will receive a letter from The Hartford mailed to your home with additional details. Keep in mind, you only have 60 days from your initial notification email to submit EOI. If the EOI process is started, but not completed or not completed at all during the 60 day period, two reminders will be sent to your email. There will be no additional time provided beyond the initial 60 day window. 

Please note that dependent children do not need to go though EOI.
