Employee must elect employee voluntary benefits to elect voluntary benefits for spouse and child(ren). In addition, spouse voluntary life amount cannot exceed 100% of the employee's voluntary life amount.

Evidence of Insurability Process for
Voluntary Life Benefits
EOI, also known as “evidence of good health” is the process by which Unum determines if you are healthy enough to be considered eligible for the amount of insurance coverage for which you are seeking. You need to complete an EOI form in the following situations:

You decline to purchase the coverage during this Open Enrollment when the plan is first made available to you, and then you decide to purchase it later

You want to increase existing coverage

You want to increase the coverage amount higher than the Guaranteed Issue (please note that dependent children are not required to submit Evidence of Insurability).
Evidence of Insurability (EOI)
Decision
After Unum reviews your EOI form, Unum will make the decision:

Approved: Your coverage will go into effect on the date of approval (you will not be billed for coverage through payroll deductions until the 1st of the month following this date)

Pended: Unum needs more information from you before they will make a decision, and they will be in touch with you to ask you for the information

Denied: Your request is denied by Unum. If you would like your application reconsidered, you may submit supporting documentation to Unum which will be outlined in the letter you receive.
